Does it cost anything?

No. The extension is free, there is no paid tier, and you do not need an account to see a score.

Why does the score show on the casino page in Firefox but not in Chrome?

Chrome does not allow an extension to read gambling sites. Firefox does, and treats a gambling blocker as what it is, a tool that protects the person using it. So the Firefox build puts the score on the operator's own page, and in Chrome you look an operator up by name instead. The session timer, the limits, the deposit budget and the block are the same in both, at version 2.5.0.

What does the extension read?

The domain of the casino page you are on, so it can look up that operator's score, and only the hostname, never the full address. If you turn on play controls it also records the minutes you spent on operators we rate and any deposit figure you type in. It never reads page text, form fields, balances or bet history, and it does not track browsing on other sites.

Is the block a legal self-exclusion?

No. An extension can be removed, and removing it lifts the block, so it is a wall, not a lock. Making a block longer is instant and ending one early takes 24 hours. For something an operator is obliged to honour, use GamStop or your state's self-exclusion programme.
